ONGOING CEU RECORD DOWNLOAD
Many
times when we receive re-cert packets, they are very thick due to the
fact that managers/educators generate certificates for every journal
in-service for each person.
Instead of using certificates for journal in-services, we have developed
what we call an Ongoing CEU Record.
Below is the link for the file which is in Microsoft Excel format. Managers/educators
can simply save this file to their computer and fill in the blanks to
create a custom file for each employee.
The only thing to remember is, when printing out this form, please print
it on facility letterhead and make sure its signed by the instructor.
